Forms to complete prior to your clinic
If you are coming for your first clinic appointment with Mr Ivanov, it will be helpful if you could bring the following information with you:

Referral Letter from your General Practitioner

List of your medications

Your pain assessment form (Lumbar spine or Cervical spine)

Completed First Appointment form
Mr Ivanov is continuously looking into the outcome of his patients and is regularly assessing their condition both before and after the surgery. This is based on Internationally recognised outcome forms which differ depending on the segment of your spine and condition.
For assessment of the outcome he is using British Spine Registry. You will find more information here.

It will be very helpful if you could complete these forms prior to clinical appointment and bring them with you both to your first and follow-up appointments.
Please note - the forms are different for the neck and lower back problems.
